club members can use this form to enter
their own orders and email them to their
club's order collator. You may email a copy
to yourself as well.
The
form requires Internet Explorer 5.0 or
above.
The form works best with Internet Explorer version
5 or above on a PC. The form will also work
for most Macintosh users using Internet
Explorer 5.0 or above, except for the
'add rows' button.
PLEASE
NOTE:
the
form asks for your FOODLINK MEMBER
ID. This is NOT
your UNFI account number. FoodLink member
IDs are assigned by your club's order collator,
and appear on the warehouse pick labels.
This is your personal ID number within your
group. Be sure to use this number (if you
use the wrong number, your order may be
lost!)
