Once
your form is complete, you can obtain your E.I.N.
in minutes by calling the Tele-Tin phone number
listed on the back of the form. The person making
the call must be authorized to sign the form and
there is a long distance charge. You can also
fax it in to complete the process.
Once you have an E.I.N. you can also use it to
apply for an authorization to accept Food Stamps.
